What it is

The documentation problem in an independent pain clinic is not “write faster.” It is “finish the note in the same mental context as the visit, without inventing a second job after 5pm.” ChartVoyant’s answer is a draft that appears during the visit, not ninety seconds after the patient has left, and a sign-off that is a clinical act — not a button a medical assistant can hit to clear the board.

On the clinic floor

Blocks stream in: history, exam, assessment and plan. Clinician-typed sentences are protected; the model does not overwrite them on the next pass. Fixture labels and placeholder exam text cannot be signed. Assessment and Plan may be blank if the clinician chooses — the software does not invent an assessment to look complete. Sign-off stays enabled while a pass is still running, with a plain-language warning: sign now to file the note exactly as it reads, or give it a moment.

Signing locks that visit’s intake, exam, and assessment & plan. They remain viewable, printable, and exportable, and an administrator can unlock them. The transcript, orders, and intake are saved with the note. After sign-off, an after-visit summary can be generated in the patient’s language, along with a referral letter or a work/school note — each editable, each released on purpose. Signing attests that an after-visit summary will be written from the signed note; it does not auto-publish clinical narrative to the portal.

What AI does, and what a clinician confirms

The model proposes wording, codes, and orders. The clinician edits and attests. There is no batch-approve for the note. Automation-bias research is unambiguous that review decays under time pressure; a one-click “accept all” would keep a human in the loop on paper and remove them in practice. We would rather be slower here.

Non-Device CDS

Suggested diagnoses, codes, and plans remain drafts with a “why suggested” trail. A clinician confirms each one. ChartVoyant does not interpret imaging or signals, and it does not make an autonomous clinical decision.

Where this sits vs bolted-on tools

Bolt-on scribes optimize for a paste buffer. The attested ChartVoyant note is the source document for the claim, the prior-auth packet, the referral letter, and the portal summary. That is the difference between a documentation product and an EMR. It is also why we refuse to ship a note that looks finished before a person has read it.
