ChartVoyant does not use numbered codes like E1234. The stable name is the word in the error — conflict, unauthorized, and so on. Banners often show Failed (409): using the HTTP status and a short sentence.
When escalating, copy the correlation id from the error if you see one. Support uses it to find the exact request in the logs without anyone sending patient details.
API error codes
Most failures in the EMR are one of these.

400 | The form failed a rule: incomplete answers, missing ICD/CPT attestation, a declined card, a bad NPI, and similar. | Correct the highlighted field. Finish required questions or signatures. For billing, attest the required codes. For payments, check the card and amount. |
unauthorized |
401 | No valid sign-in: wrong password or two-factor code, an expired invite/reset/QR, a revoked session, or an inactive account. | Sign in again. For two-factor, use a fresh authenticator code. Expired links need a new one from the front desk or an admin. If the account is inactive, ask an admin to re-enable it. |
forbidden |
403 | You are signed in, but this action isn’t allowed — wrong role, the practice is suspended, or a second factor is required for controlled substances. | Use an account with the right role (Admin → Roles & permissions). If the practice is suspended, call ChartVoyant support. For e-prescribing controls, complete the extra authenticator step. |

409 | This change collides with what’s already saved: a duplicate name, a concurrent edit, a payment already voided, or a job already running. | Reload and retry with the latest data. Pick a different unique value if one was taken. Don’t retry a payment or a migration commit blindly — check the current status first. |
rate_limited |
429 | Too many requests in a short time (sign-in, two-factor, uploads, kiosk, or the app as a whole). | Wait about a minute and try once. Close extra tabs that may be refreshing. Don’t keep retrying immediately. |
internal |
500 | An unexpected server problem. The message is generic on purpose so patient information is never leaked. | Copy the correlation id if you see one and send it to support. Retry once; if it persists, call (855) 686-9268. |
service_unavailable |
503 | A service ChartVoyant depends on — often AI — is not available right now, usually because of configuration. | Read the full message; it usually names what to fix. If you can’t, call support and quote that sentence. |

Fax failure codes
When an outbound or inbound fax fails, the queue may show one of these next to the vendor’s own words.

An unclassified fax failure. | Call support with the time and destination (no patient details). |
